Voter Information Card

Voter information cards are furnished by the Supervisor of Elections to all registered voters residing within their county.  The card contains information regarding a voter's registration record, such as registration number, registration date, name, party affiliation, birth date, residence address, precinct number, polling place address, etc. The supervisor may include additional information on the card if he or she deems it necessary.  

Voter information cards are NOT required for identification to vote at the polls.  Instead, at the polls, you will be asked to provide a valid picture identification with signature.  A voter information card is strictly for information purposes only, not for identification.  

A voter may receive a free replacement voter information card at any time by contacting the Supervisor of Election's office, or completing the Online Florida Voter Registration Application Form. Upon verification of registration, the supervisor will issue the duplicate card.  

In the case of a new registration, a voter information card is mailed to the applicant after the registration is approved.  This voter information card constitutes notice of approval of registration. Cards are normally received by the applicant within two weeks of our office receiving the application. If the applicant is not yet 18 years of age, the card is mailed after the voter turns 18. If the registration application was incomplete, the Supervisor of Elections will send a notice to the applicant requesting additional information on an application form. If the application is denied, the Supervisor of Elections will send a notice to the applicant notifying them of the denial and the reason it was denied.  

In the case of a registration record change, the Supervisor of Elections will issue the voter a new voter information card upon verification of previous registration.  A new voter information card replaces all previous voter information cards.  

Florida Statute 104.013 states:

(1) It is unlawful for any person knowingly to have in his or her possession any blank, forged, stolen, fictitious, counterfeit, or unlawfully issued voter information card unless possession by such person has been duly authorized by the supervisor.
(2) It is unlawful for any person to barter, trade, sell, or give away a voter information card unless said person has been duly authorized to issue a voter information card.
(3) It is unlawful for any person willfully to destroy or deface the information card of a duly registered voter.
(4) Any person who violates any of the provisions of this section commits a felony of the third degree, punishable as provided in s. 775.082, s. 775.083, or s. 775.084
